What does a shadow on lung mean?

Someone I know coughed up blood was sent for an xray found a shadow right lung and was put on antibiotics he has no cough is the fittest person i know never drunk never smoked runs 5-10 miles regularly he has to go for repeat xray but in couple of months not fastracked I don't understand how there could be chance of cancer
